Job Description

💼 Job Type: Full-Time

 

Palliative Care - 3 positions

 

Chronic Illness Home Care - 2 positions

 

📍 Location: Islandwide

 

🕒 Work Schedule: Mon to Fri | 8:30 AM – 5:30 PM

 

💰 Salary Range: Up to $4,500 per month

 

🎁 Allowances: On-call allowance + Fixed monthly nursing allowance

 

 

 

👩‍⚕️ Key Responsibilities

 

• Conduct home visits for patients with chronic illnesses

• Perform comprehensive nursing assessments and coordinate with doctors for follow-ups

• Monitor medication compliance and support patients in managing long-term conditions

• Perform nursing procedures such as wound dressing, catheter insertion, and NGT care

• Initiate, review, and implement individualized nursing care plans

• Maintain accurate documentation and update records in the IT system

• Identify and refer patients/families for financial or psychological support when needed

• Participate in community activities when required

 

Job Requirements

🌟 What We’re Looking For

 

Palliative Care - 3 positions

• Min 3 years nursing experience

• Diploma/Degree in Nursing or equivalent

• Registered Nurse with the Singapore Nursing Board (SNB)

• Possess a Nursing Certificate in Palliative Care

• Compassionate, patient, and able to work independently with strong communication and assessment skills

 

 

Chronic Illness Home Care - 2 positions

• Min 1 year nursing experience

• Diploma/Degree in Nursing or equivalent

• Registered Nurse with the Singapore Nursing Board (SNB)

• Compassionate, patient, and able to work independently with strong communication and assessment skills
