Job Description

Salary & Benefits

💰 Up to $4,000 basic per month + Allowances

📍 East Singapore

🗓️ Monday to Friday

⏰ 8:30 AM – 5:30 PM

Key Responsibilities

  • Conduct home visits to provide palliative nursing care for patients
  • Perform comprehensive nursing assessments and develop individualized care plans
  • Administer and adjust palliative medications under the direction of doctors to manage symptoms effectively
  • Work closely with the Medical Director, doctors, and multidisciplinary healthcare teams
  • Educate patients, caregivers, and family members on nursing care and symptom management
  • Coordinate referrals to inpatient hospices and other healthcare or community support services when required
  • Identify patients and families requiring financial assistance or counselling support and facilitate appropriate referrals
  • Provide emotional, psychosocial, and bereavement support to patients and their loved ones
  • Complete a minimum of six home visits daily
  • Participate in community outreach and fundraising activities when required

Job Requirements

Requirements

  • Must be registered with the Singapore Nursing Board (SNB)
  • Diploma or Degree in Nursing or equivalent
  • Minimum 1 year of relevant nursing experience
  • Experience in palliative care, hospice, home care, or end-of-life care is highly preferred
  • Experience with symptom management and titration of palliative medications (e.g. morphine, fentanyl) will be an advantage
  • Compassionate, empathetic, and committed to delivering patient-centred care
  • Strong communication and interpersonal skills
  • Comfortable travelling for home visits
